taxable reportable income and will be reported to the State Controller's Office.
Tax deductions will be reflected on monthly payroll checks following the date incentives
are disbursed. Participants who wish to decline financial awards can do so by selecting
the “Decline Rewards” option at the end of the survey. Prizes are non-transferable.
All prizes will be processed and distributed approximately during the week of January
7, 2008. All reportable taxable income will be recorded in January 2008 payroll,
and recorded as taxable income in 2008
Prizes not claimed by December 31, 2007 will result in forfeiture.
